Name/Situs Update Form

To submit a request for a change in records regarding a Name misspelling, Name change, or Situs Address Correction, you can do so using the electronic form at:

**Please note: Situs address is the physical address of the property. This is not the form to change a mailing address. This is to correct an error to the physical address of a property, (e.g., missing number).

The form can also be found on the WCAD’s Homepage, under the E-Services tab.


 Select the appropriate box(es) for your reason(s) for change. The form will adjust based on your selections.

 

 Documents can be uploaded in the “File Upload” field.


Click “Submit” once you have completed the form.

**This form will be submitted electronically. You do not need to print this form for it to be submitted.**

A PDF copy will be sent to the email address you provide in the form, confirming receipt of your request. 

Expected processing time: These requests are processed in the order received.
