If this is the first rescheduled hearing request and is made prior to the date of your scheduled hearing, you may use this form to make your request:
If WCAD has already processed a reschedule request for your account, please use the 2nd Reschedule Request Form which will require ARB approval.
Please read the instructions clearly. Check the email you provide on each form for follow-up.
