Form for requesting an ARB hearing reschedule
If this is the first rescheduled hearing request and it is made prior to the date of your scheduled hearing, you may use this form to make your request:
If WCAD has previously processed a reschedule request for you, or if you are requesting a reschedule on the day or days after you were scheduled, please use the 2nd Reschedule Request Form which will require ARB approval.
