To request a Confirmation of Exemption, you may use the link provided below. The form can also be found on the WCAD’s Homepage, under the E-Services tab.

Click in the Reason for Contact field and select Request a Confirmation of Exemption from the drop-down options.



The form will automatically adjust depending on your reason for contact.

When you have completed the form, click the Submit button at the bottom of the form:

 
**This form will be submitted electronically to the Exemptions Department. You will not need to print out this form to be submitted.**
